Highlights: A free, six-month health coaching and remote monitoring program for your patients with chronic obstructive pulmonary disease (COPD) and congestive heart failure. Physicians can refer patient by completing and faxing the LHIN-specific form by location. Patients can also self-refer themselves to the program as well.
